Project Application for Research with Human Subjects
All human subject research conducted by Texas A&M Health Science Center employees or students must be submitted to the Institutional Review Board for review.
The forms on this page were revised for use beginning November 1, 2011. Please discard all old forms downloaded previously.
Submit all completed and signed forms and documents to the Office of Research Compliance in hard copy, single sided.

CITI Training
All individuals involved in research project involving human subjects are required to be trained via the Collaborative Institutional Training Initiative (CITI). This is an online training program that can be accessed at any time from any computer with the login and password that is established when you register at the site. It is important that during registration you select the Texas A&M Health Science Center and not the University or your completion records will not be sent to our office and you won't be cleared to participate in research.
