Please keep in mind the following:

• Applications are returned to:

                         Office of Academic Records
                         Baylor College of Dentistry 
                         3302 Gaston Ave.
                         Dallas, TX  75246

• An application fee of $35 must be sent with the application.
• Reference forms from three persons must be sent to the Office of Academic Records directly from the recommender.
• Transcripts from all colleges and universities attended must be sent directly to the Office of Academic Records.
• Graduate Record Exam (GRE) scores are required.
• International students must submit TOEFL scores.
• Include a copy of your Curriculum Vitae or resume and a narrative about your goals and objectives.
