Deirdre Hello, I was diagnosed as having LP on my skin on April 1 (ha) of thsi year. The following week my dentist identified OLP in my mouth -- just teh white lacey stuff, on my inner cheeks. I have been using topical Liderm cream on the skin lesions, and Reactine 1x daily, and Benadryl as needed (all drug names are in Canadian versions), and this has been helping my symptoms. I am seeing an oral pathologist tomorrow, and anOBGYN next week (hopefully to rule out LP in the gen

DrRees Debs, ok. There is really no reason to think that orabase alone will do any good. Protopic helps some people but not all. When you mention prednisone to me that means taking prednisone tablets. That is considerably different from using topical corticosteroids and the risk of side effects is minimal.

Debs My docs don't know what else we can try, and I don't want to try prednisone unless it's the only option left.
Deirdre My question is, what can be done for PREVENTION? I sthere any known protocol? I understand that learning new mechanisms to cope with stress may be useful, however I am currently experiencing a new outbreak of skin lesions (also responding well to the Liderm/Reactine.Benadryl treatment), and I must say,, it is disheartening to be told by my dermatologist that I am doing great, even as my skin is itching ferociously from head to toe.
DrRees Ajit, yes metronidazole and sulfasalazine have been used for severe cases and they work sometimes. I don't know anything about using anti-helminthics or lactisyn.
Olive Re:Prednisone. Is a Prednisone lavage (mouthtwash)daily appropriate maintenance therapy?
drburkhart Deirdre, have you been able to trace the out breaks to any specific activities?
bonie I also have LP of the vagina and a biopsy was done in my throat and it is there too, but it is my mouth and botom lip that drives me insane, it is so painful, I need help.
DrRees Olive, Yes it has been reported to be of benefit. The trouble with it is that it washes out of your mouth pretty quickly. If it is controlling your olp then by all means use it. It would probably be best to spit out rather than swallow.

Olive Yes, I spit. Problem is I'm not getting the relief I did at first so thought perhaps it's not even a good thing.
drburkhart Bonie, with regard to your lip, do you drink hot tea or hot liquids. do you cause any trauma to the area?
Debs I have found that taking a Zyrtec every night helps alot with itching, nothing helps with burning or growth.

bonie The only thing I'm using now is Lidex for my mouth and lip and Lidex E for my vagina. Yes I drink coffee but the lesions are so bad that anything that touches my lips make it crack and bleed.
Deirdre The diagnosis is so new (six weeks) that I haven't really ha a chance to track patterns. It started in the fall, on my ankles then wristrs, and I was told (by dermatologist ) in January tahtit was "dry skin." Th elichenoid lesions erupted in March and looed odd enough that I went bcak to the derm., who told me that until these characteristic bumps appeared, a diagnosis f LP would have been almost impossible. I believe her. I developed shingles just before Easter. I tho

drburkhart Bonie, I have gotten some emails from patients who have LP on the lip and they reported that a cup that is hot would tend to make the area worse.

drj i don't do a lot of research on lp here at the dental school, but i attended a recent oral medincine meeting and there is a lot going on in autoimmune diseases, which will be applicable to LP. most of the reseach is just that, research and is not ready for general use.

DrRees Ajit, not as much as it is in your part of the world but we do not close our minds to its usefulness. The problem is that often the claimed results aren't the same when the drug is studied under controlled circumstances.

drburkhart Ajit, interesting question. General Dentisitry did publish the aloe vera article, but again it was a small case presentation. Others may be doing more research on the subject that has not been as of yet published. I do get emails from patients who are trying different alternatives.

DrRees Barry, you are right in that there is a relationship. Many people with OLP use topical steroids that tend to allow yeast to overgrow. Sometimes the lichen gets better but the yeast gets worse and vice versa. Your care provider can help you monitor this and I'm sure does.

drj people ask about homoeopthy and natural products, but not a lot is know. if patients have found some integrative approach that has worked for them, that is great, but, usully there is little in the literature to support its use in LP. i know of one study looking at curcurum, the active ingredient in tumeric,(the yellow root used inn curries) using it for its antiinflammatory effect, but the study is ongoing and in the first stages.
DrRees bonie, we would rather talk with your doctor because we can't really know for sure what your condition is without seeing you.
Deirdre Can you pleasee if you know of any reseach into prevention? My OLP so far is not causing me pain and I hope to keep it that way. Will the Reactine (I use it in the morning, it's te same antihistamine as Zyrtec) and Benadryl as needed (if the itching drives me to distraction, esepcially at bedtime) perhaps going to help keep the OLP from worsening, even as it helps me cop ewith my skin symptoms? Also teh LP feels VERY ALLERGIC to me. I have allergies to pennacillin and

drburkhart Debs, Dr. Rees and I were just talking about getting some of our data together from our survey and we could look for some patterns. There may even be some patterns with the way that people deal with chronic type diseases.

Ajit I agree with the need for controlled studies. Presently one of my post graduate students is doing her M.D.S. Dissertation comparing steroidal, non steroidal sequential integrated & placebo therapeutic programs in the management of olp . The study should be completed by December 2003
DrRees Margit. Prednisone does work but as you know you shoouldn't take it unless you need it. We have had good success using topical steroids in the mouth instead of taking prednisone tablets.

DrRees Renee, it happens but that is pretty rare. Mostly it is on the skin, and in the mouth and sometimes the genitalia.
Deirdre I am intrigued by someone's mention of candida. I had recurrent yeast infections whiel breastefeeding my daughter, andhad to take diflucan to get rid of the pain. Is there a known candida/LP link? Shoudl I discuss this with my oral pathologist? Did I mention that I see her fo rthe first time tomorrow, and I am hoping to have intelligent questions. I''m seeing Dr. Linda Lee, at Princess Margaret Hospital in Toronto.

drburkhart Renee, Dr. Eisen has been a guest so be sure to read his transcripts on the subject. Yes, it can occur and is often not considred unless the patient tells the OBGYN that they have LP elsewhere.

drj LP can appear on any skin surface and especially any specialized mucus membrane surface. that includes the mouth the vigina and the urinary tract in males.
DrRees Deirdre, Please don't misunderstand. Candida doesn't cause lichen planus but sometimes the treatment of lichen planus can cause candidiasis. So can a dry mouth which seems to be a problem for some of our patients.
drburkhart Ajit, I have been telling patients this for years. It is a good coping mechanism and stress reducer. I find that most patients do not know how stressed they really are until they reach a state of "no stress" in which to compare.

Deirdre DRJ, i had a problem wit hanal fissure about a year ago year ago, and my asthsma specialist is treating me for GERD. Could this be related to LP?
DrRees Debs, not that I am aware of.

Ajit As a lot of Urinary tract discussion is taking place , Sir would you let me know whether it is also your observation that OLP patients often demonstrate high Serum Uric Acid Levels ?
DrRees bunny, yes we know. That happens to our patients too. What we usually do is stop the topical steroid for 3 or 4 days and only use the yeast medication then start back using both. That works most of the time but not always.

Deirdre Should I be worried if my mouth is feeling "tender"? Are here things I can do to keep OLP from progressing?
bunny I find that not stressing out about olp is the best medicine.
drburkhart In general: If you have OLP, and you go in for eyes, ears, vaginal, esophogeal problems , it is wise to let your healthcare provider know in detail your history.
DrRees Renee, yes that happens maybe more often than we know. A mild case of olp is more likely to come and go but we tell our patients that they should anticipate a recurrence at some time in the future so they should be prepared to treat it if necessary.

drj i know of no other real direct medical connections between LP and other diseases, except other autoimmune diseases. but certainly if one has LP it does not mean that you are free from getting any other diseases. but it is often frustrating and confusing to try to unify multiple medical problems into one disease.
Ajit Do patients in the USA get suggestions to switch over to a Vegetarian Diet -- particularly avoid eggs/ red meat ?
Deirdre Thanks, Dr. B. I find that some of my medical team are a bit skeptical about LP as a potential factor. How can I make sure that the whole picture does get investigated? Particularly since one's body is staked out in pretty well-defined territories by medical specialty?
DrRees Margit, Biotene is certainly a mild toothpaste and it doesn't have all the ingredients some other toothpaste do. That suggests that you may need to be selective with your choice of paste and there is nothing wrong with using Biotene indefinitely

bunny Also some of the readings are frightening re olp leading to cancer. We need some clarification and alleviation of fears.

DrRees bunny, If there is any increased risk it is slight but it means that you should continue to be followed by your care provider so that any unusual changes can be detected very early. Try not to worry about that but also do try to get your olp under control.

Ajit I am a Professor at the Goa Dental College & Hospital teaching Oral Medicine & Radiology. We do get a lot of OLP cases & have found Serum Uric Acid Levels high in them. Switching to a Vegetarian Diet has also helped-- particularly avoiding eggs & red meat.
drburkhart Bunny, any time you have chronic irritation (raw, bleeding surfaces that do not heal), you need to be checked on a regular basis. That way if there is a problem, it can be discovered early.

DrRees Margit, yes, topical corticosteroids are safer and Protopic has received good reports but some patients complain that it burns when you use it. There are one or two new drugs that are not steroids that may help but so far they haven't been studied for olp nor approved for that use.
bonie They now have a biotene gel for dry mouth use, I use it every night along with the toothpaste and mouthwash
Debs Is there any evidence that with recurrent lp over time it will get worse and worse?
drburkhart drj, are you seeing a genetic factors? We know that OLP is not contagious, but I have been receiving emails lately that a sister, mother may also be uder treatment. Are we just reporting this better at this point?

drj another topic/question i get from patients is what caused this to start and they often have something that they associate the onset of their LP. there is no know cause, you didn't get it from anybody and you can't give it to anybody. but there is often some event that happened coincidental? to the onset that an individual thinks may have started it.

drj as far as genetic, some researchers feel that all human diseases have a genetic association, except accidents. but what the connection is, is not known. and the genetic part probably has to be triggered by some event, for example a transient odd viral infection, before the disease starts. there is a lot left to know.
Deirdre Dr Rees, I had assumed that the hockey was a good thing, and in fact have signed up for summer league (am misisng the exercise dearly). Dr B, the LP started on my ankles, moved to wrist, crept up my legs, and then started on my back. It is now popping up on my abdomen. I did switch detergents (had used Tide Free whiel my kids were infants/toddlers, but in th epast year had been cheaping out). Have gone back to the detergents included in my babying regime. drburkhart Deirdre, Just a note to answer you and for everyone else also. I think that with chronic lichen, it is a good idea to keep a journal and read over the material from time to time. Sometimes you may see patterns that you would not otherwise notice. I do believe that there may be trigger mechanisms just as Drj has mentioned.
Ajit Deirdre may be asked to consult a Homoeopath with the Homoeopathic medicines suggested earlier in this chat . His signs are typical & the lesions would start healing in the reverse order of their occurance
DrRees Margit, usually you can't find out everything that is in a toothpaste but we know that biotene does not have something called sodium lauryl sulfate that can be an irritant and we don't think it has cinnamon in it.
Deirdre Drs, I've seen references to Biotene but haven't seen it on Canadian drugstore shelves. Is it an Rx product, a health food product, or something I might find at the Giant eagle in Buffalo?

DrRees Deidre, Giant eagle probably has it but I'll bet it available in Canada. It is over the counter and if you ask your pharmacist he/she can order it for you if they don't stock it.

drj biotene is made by the laclede inc. in the los angles area. you should be able to find them on www.google and get locations where you can find the produce and all the ingredients.

Debs Is your data that cinnamon is an irritant or causation?
DrRees Loraine, the problem is really something called cinnamic aldehyde that is in cinnamon. You can find out if it is a factor with you by avoiding it for a couple of weeks.

drj cinnamon is the most common oral allergen. if you are using a cinnomon containing product you should stop for 2-3 weeks to see how your mouth responds. depending you can restart to see if problems arise
DrRees Deidre, we are evaluating that right now. The problem is that other flavoring agents such as peppermint, seems to be cross reactive in someone who is allergic to cinnamon products.
Fred I've been watching the discussions and think I might try Biotene for a mild OLP that may be triggered by HOT drinks and spicy snacks... I didn't like the Lyderm treatment.
Ajit Is Denta-Care / Meswak / Vicco Vajradanti tooth paste available in the USA . These are all Ayurvedic preparations. Also is Chandrika / Medimix/ Calendula Toilet Soap used to reduce skin itching in OLP cases in the USA ?
Deirdre I had to kick mint toothpaste whiel pregnant but used cinnamon CloseUp. I'm not sure what oter flabours there are, aside from bubblegum and flouride-free grape.
bunny I have read that olp usuallu clear in about 18 months. Any truth to that?
DrRees Deidre, there are some review articles that outline most of those. Contact Dr. Burkhart later and I am sure she will be willing to send you some material.

DrRees Renee, no physical effort won't make your lichen planus worse unless you scratch a lot after you sweat. You should try to avoid scratching a lichen lesion because that can spread it.
Debs Is there anything we as patients can do, to assist in research for prevention?
DrRees Debs, If someone is conducting a study in your area, participate in it.
Loraine1 What about brushing teeth? My oral hygenist insists I brush my gums and rince with Lysterine as well as floss. Sometimes that hurts.
Ajit Drs. do recommend Ayurvedic Soaps to reduce itching. Asking the foam to be left on for 10 minutes
Deirdre DR REES: I didn't realize that one could actively help LP to spread. I have been avoiding scratching to avoid scarring. I thought it just popped up. Can I be making it spread? How?
drj the denta-care product contains tea tree oil or melaluca oil as the active ingredient (and cinnomon as a flavor) the tea tree oil commonly causes an allergic reaction. the oil is used as a disinfectant. but since LP is not an infection, i would not expect it to be of any value and has the potential to be a problem.
drburkhart Deirdre, I think that lifestyle changes are a big factors. Look at it this way, it won't make things worse and you could live longer as a side .
DrRees Deidre, you are right it can become quite severe. Try to control it while it is mild and usually that won't become a problem. Discuss this with your care provider.
Deirdre Re; Control. I hope the oral pathologist will have concrete info on that. I won't see her until tomorrow. If I have questions I will send them to Dr. B.

DrRees Deidre, scratching is well know to cause spreading along the scratch line. That's why you often have it on the inside of your cheeks along the chewing line.
