burkhart Boots, Usually the lesions are purple-brown, flat and may be single or multiple.
burkhart Jacky, How do you know that you have oral lichen planus?
boots I have not yet had an oral biopsy to determine if I do suffer from lp but I have not had an oral breakout since I began using biotene toothpaste and other gentle oral cleansing products. However, right now I have a breakout on my hands that is red and presents on the joints and is round and flat.

jacky i have not but my mother she has it for two years now and the docters donot know what to do she has tryed so many medicines,the last one wasneotygason

burkhart Jacky, Sometimes patients have allergic responses to dental products and additionally, the patient may have more ulcer-like areas when the oral lichen planus flares.

rees Type HEREJackie, this is Rees. I work with Dr. Burkhart. It sounds as though you may have had an irritant or allergic reaction to a toothpaste or other oral hygiene product instead of LP. What kind of toothpaste were you using before? Also, we are not familiar with neotygason in this country.

rees Boots, it is fairly easy to biopsy a skin lesion and the likelihood of a good diagnostic report is better than on the oral mucous membranes.

2sweet its ok to use ...doesn't stay on for long....it seems to take a little of the pain away

rees Jackie, first there are some studies that suggest that synthetic vitamin A (retinoids) will help some forms of LP but occasionally make other forms worse. The only thing to do is try the neotygason and see how it works. Second, what type of prosthesis does you Mother have? Is it made of metal or plastic?

plemons 2sweet, it often takes a couple of weeks for topical steroids to begin to work well. How does it taste? Many of the medications we use taste a little bitter.

rees Jacky- good, that is not very likely to cause an allergic reaction but sometimes other parts of the prosthesis besides the teeth have other materials in them.

2sweet there is really no taste and that is very nice. I wanted to try Lidex, but my dentist said he could not find it for oral use.
boots does lp affect the joints internally? As in arthritis.

burkhart Boots, We have found that patients with lichen planus fall in a certain age group-usually 50's and this age group also tends to have more arthritis.

2sweet I wish my Lichen Planus had waited until I was in my 50's(I'm 31)...but I also have the startings of arthritis

rees Jacky. I may be repeating a bit but the first step is to have your Mother receive a biopsy to confirm LP. If that is what she has we would treat her with a topical antiinflammatory agent (corticosteroid) That usually will give some relief but there are a variety of them and some are more effective than others. If the topical steroid doesn't work then there is a possibility that other medications she is taking should be changed. The type of steroid we usually sta

plemons 2sweet, some medications used to treat both arthritis and diabetes can occasionally cause lichenoid reactions. Can you correlate the beginning of your lesions with any medications?

2sweet No ...pretty positive on that. It seems very unrelated to anything.
LRB My LP is in its early stages, so no biopsy. Sometimes my gums are sore. Can that be LP or something else, possibly? How can you tell?
burkhart LRB, There are studies being conducted that evaluate the trigger mechanisms of lichen planus. We are currently evaluating lichen in a retrospective study, but are not far along in these efforts at the present. I am especially interested in lifestyle and stress factors.

rees LRB- there are a number of so-called mucocutaneous (skin and mucous membrane) diseases that can look like LP in the mouth. That is one of the reasons that a biopsy is so important. We often see patients who have been under treatment for years for the wrong disease.
burkhart LRB Yes, you really need a biopsy in order to know what type of treatment is beneficial. Many mucosal disorders appear similar clinically.

LRB I have the white lines inside my cheeks, so that is why I think my periodontist said I was in the early stages--he said too soon to take a biopsy, and said nothing on my recent visit.
burkhart LRB-We have talked about this idea and would like to conduct this research, establishing a data base; as is the usual case, the element of funding and resources is a problem, but we are working on that aspect.
rees Jackie- I agree, it is hard to read this since we are talking with a number of people at the same time. If you would prefer to contact us at the web-site soon we will be happy to try to answer your questions more clearly.

boots I didn't know there were stages to lp. Is that right?
LRB I've had "flare-ups" but none worth mentioning since giving up Listerine and being careful of mint and spicy foods.
burkhart LRB, This is good. At least you are not having any discomfort. We strongly advise that lichen planus is monitored several times a year and more frequently if you are having problems.

2sweet Yes- exactly
plemons Boots, there are multiple forms or clinical presentations of lichen planus in the mouth. It ranges from white, lacy lines to white patches to red areas to ulcerations or blisters like 2sweet just mentioned. They are not really stages because one doesn't necessarily lead to another. In fact, most people have more than one kind of lesions at a time.
burkhart LRB-Yes, we hear these comments from others. Usually a log of trigger mechanisms is beneficial.
rees Boots- there is a blistering form of LP but it is very uncommon. Some of the other mucocutaneous diseases are more likely to be associated with blisters.

boots 2sweet, sorry I'm not certain how to answer you. I have had ulcers in my mouth but only this past February did I ever hear of LP. I have not had a flare up since and did not get a biopsy since the episode ended in 5 days.
rees 2sweet- sounds like it is LP so hang in there and try various treatments until the one that works best for you can be determined.

boots I currently have a small flare up on a couple of knuckles of my hands. These bumps only last about a day. I notice them because they hurt when I move my hands.

burkhart Oral photographs allow the patient and practitioner an exellent opportunity to compare the lesions and status of the oral condition on subsequent visits.

rees 2sweet. In almost every area of the country there are some health care providers who know a lot about LP. The ones we know are mostly dentists but if you'll contact us later we will be happy to try to match you up with someone within a couple of hundred miles of where you live. We also would be more than happy to talk with any of your health care providers about the contidion.

Herb My lesions only hurt sometimes, usually when I eat things like onions, garlic, or sometimes just the flowery looking weeds that grow in my yard
LRB If lesions are between your teeth and the perio decides a biopsy should be done, is tooth removal necessary to get the biopsy they need?
burkhart Jacky, we really do! In the last week we have had inquiries from several countries .

plemons LRB, tooth removal is certainly not necessary. Only a small bit of tissue is needed during the biopsy.
Herb My doctor got me gel to put in my mouth when they hurt - is that what I need? Sometimes it helps, but usually it doesn't.

rees Herb- There are several types of gels. We use a steroid get quite often to treat LP. Do you know which one you have been prescribed?
LRB I've notice a white line, similar to the inside of my cheeks, at the floor of my mouth. Is that common? Bumps, but no sores.

plemons LRB, white lesions of lichen planus can occur anywhere in the mouth including under the tongue. However, it is possible to have other things as well. That's why it is important to be followed by a dentist who is used to seeing patients with lichen planus.

rees Herb- That is often very effective and in fact that is what we use the most. If it is effective in controlling your pain you might talk with your doctor about using it a bit more often for several weeks. Sometimes that makes the lesions go away. We however, have no cure for Burt Reynolds movies

Herb I saw a dentist once, who didn't have lichen planus, but he did have an obvious finger infection.
rees Herb- I think only your dentist or physician can answer that for sure but LP can occur anywhere in the mouth and on the skin. It can even occur on the genitalia. If you are asking about the relationship with planters warts there is no known relationship that we are aware of.
burkhart Herb, As we mentioned earlier, you really need to be monitored with any oral lesions at least twice a year and more often if you are having prolems.

rees 2sweet. That is a common comment but there is a huge body of literature describing its safe use in the oral cavity and on other mucous membranes. If you have a particular pharmacist of physician who would like information about that literature let us know and we'll try to help.
