My sister has serious periodontitis. I have read about a new treatment using minor surgery and injection of amelogenin gel under the tooth which causes the gums to grow back and new ligaments to form. Do you have any information about this - where treatment can be got in the UK (Europe) - and where more info. can be found?

This new product is marketed by Biora AB, Malmo, Sweden as an enamel matrix derivative to encourage periodontal regeneration in the treatment of periodontal disease. It is primarily composed of the hydrophobic protein aggregate derived from fetal porcine formed prior to formation of the hard tissues on the dental crown and root.  It is an active protein in the embryogenesis of cementum, periodontal ligament and alveolar bone. Therefore, it is believed to have value in promoting regeneration of periodontal supporting tissues in humans.  Studies performed in vitro and in vivo with animals were encouraging and showed the ability to produce new attachment apparatus.  These are summarized in a recent supplemental issue of the Journal of Clinical Periodontology, Vol. 24, No. 9, part II, September, 1997.

Human data is very limited and clinical trials are in progress to study the efficacy of this product in therapy for periodontal patients.  The material is used in conjunction with periodontal surgery; after flap reflection and debridement of the defects, and definitve root preparation which includes root planing then acid etching the root surface to remove the smear layer, the Emdogain product is mixed with propylene glycol alginate as a vehicle (allowing 5 mins for dissolving of the enamel proteins) and applied to the root surface to cover the entire exposed root surface prior to flap replacement and suturing.

Once again the available clinical efficacy of this product is still being studied and we have not begun using the product at this time at Baylor College of Dentistry, Graduate Periodontics clinic.
